You will meet with injured workers in the field and assess the medical and/or vocational components of injured worker's situation to restore injured worker's function, capacity and self-esteem.-- I addition to a competitive base salary, our unique commission plan rewards the FCM based on the quality and completeness of their work/documentation, for hours worked over 40. This promotes a great customer service and value to our clients. Home office, cell phone, mileage , salary with monthly bonus potentialMAJOR DUTIES AND RESPONSIBILITIES: Assesses and analyzes injured work! er's medical and/or vocational status and develops a viable, r! ealistic service plan. Meets with clients in their homes, work-sites, physician's or therapist's office to provide on going case management services. Develops a plan of care for patients from admission to discharge. Monitors patient progress toward desired outcomes through assessments and evaluations. Communicates both in-person and telephonically with client, medical providers, attorneys, employers, insurance carriers, etc. Develops and administers education and prevention programs. Develops guidelines for working with patients, healthcare management companies, insurance companies or other third party payers to determine quality, cost-effective care. Applies all special instructions required by individual insurance carriers and referral sources. Applies all laws and regulations that apply to the provision of rehabilitation services. Testifies when called upon to do so, to substantiate any casework or reports that may be seen as relevant to hearing involving client. Evaluat! es outcome of patient care. Prepares reports and other required paperwork as documentation of all casework activities at May arrange referrals, consultations, therapeutic services, and confers with other specialists on course of care and treatment. Attends staff meetings Participates in sales activities.

Job RequirementsQualifications:
BI LINGUAL SPANISH REQUIRED
Must have current nursing license in the state designated, Preferred: workers' comp experience, case management experience, CCM, COHN, CRRN, CDMS encouraged., must have current drivers' license and be willing to travel, must be computer literate. EDUCATION/CREDENTIALS: Licensed RN in accordance with state laws of jurisdiction. JOB RELAVENT EXPERIENCE: 2 - 4 years case management experience .

Qualificati! ons:
JOB SPECIFICATIONSBi lingual Spanish requiredØ Registered nurse with current state (New Mexico) RN license.Ø Previous (3 or more years) case management experience required.Ø Bachelor's degree or equivalent experience preferred.Ø CCM, CRRN, COHN, or CDMS eligibility or current certification encouraged. These designations arerequired where dictated by state law.Ø Knowledge of laws and regulations governing delivery of rehabilitation services.Ø Knowledge of workers' compensation and disability industryØ Strong problem solving and analytical skills.Ø Ability to work independently, requires working from home office.Ø Strong computer skills with MS Office: Word, Excel and Outlook. Requires the use of a laptop computer and smart phone. Able to complete automated files.Ø Regular, daily local travel required.Ø Professional communication skills both written and verbal.
